including unlimited use of the cardiovascular equipment.
Sessions are either One to One for individual
packages, or One to Two for Two Person training
packages.

THERAPY:
Prescribed Treatment (Insurance
Reimbursed):
Many of our therapy services may be covered
by insurance. We accept many types of insurance, and must
pre-authorize all insurance coverage with the insurance
provider. Every case is unique and coverage depends on your
specific policy. Insurance will only cover therapy services if
a specific injury or health problem exists which will be treated,
and treatment is medically necessary. A physician prescription
is required. If therapy treatment is authorized for insurance
reimbursement, we will bill your insurance company the usual and
customary fees based on industry and insurance guidelines.
Non-Prescribed Treatment (Out of Pocket):
Out of pocket Massage Therapy fees are all based
on a sliding scale, at discounted rates (from the usual and
customary fees used above), depending on the package of sessions
selected. Method of payment includes cash, check, or Credit
Card ( MasterCard, Visa, American Express, Discover ). All
checks must be made payable to HealthQuest Fitness &
Therapy. Fees for out of pocket therapy is listed below.
